Apply for a U.S. Passport

Southeast Branch Library is an official passport acceptance location. Appointments required, except on "walk-in Wednesdays." Please allow 30 minutes per person for the passport application process. If you have multiple people applying with you, please make separate appointments for each person. 

Passport Services

New Passports

  • The library processes new passport applications. At your appointment, staff will verify the information of adults and children using Form DS-11. Staff must witness your signature. We then send your application and supporting documents to the State Department for processing. 

Renewals

  • Library staff will gladly look over your renewal form as a courtesy, but you must mail your own renewals. Please see the U.S. Department of State’s Renew by Mail for information.

Replacement and Name Change

  • The library has the correct forms for passport replacement and name changes. Staff can help fill out these forms, but we do not process these applications. 

Before Your Visit

  • All applicants must bring their own 2"x2" passport photo. The library does not provide photo services.

What to Bring With You

  1. Completed Application for a U.S. Passport (DS-11)
    • Fill out the form online on the State Department website. Print out your completed form. If you must fill out the form by hand, write legibly and use black ink only.
    • Forms must be filled out before your appointment.
    • Do NOT sign the application until you are instructed to do so by the passport agent.
  2. Proof of U.S. citizenship: Bring the actual document and a black and white photocopy of the document. See a complete list of accepted documents. Examples:
    • Certified U.S. Birth Certificate
    • Undamaged U.S. Passport Book or Card
    • Certificate of Citizenship
    • Naturalization Certificate
    • Consular Report of Birth Abroad
  3. Photo Identification: Bring the actual document and a black and white photocopy of the document. See a complete list of accepted documents. Examples:
    • Valid Driver’s License or State I.D.
    • Naturalization Certificate
    • Government I.D.
    • Military I.D.
    • Previous undamaged U.S. Passport Book or Card
  4. Passport Photo
  5. Fees: There are two separate payments required for a new passport. 
  6. Additional Rules for Minors: 
    1. Passports for Minors under the age of 16 
      1. The minor must be present. 
      2. Both Parents or guardians should appear with the minor. If both parents or guardians are not able to appear, please complete form DS 3053 and have it notarized prior to the appointment.

    2. Passports for Minors aged 16-17 
      1. It is recommended that at least one parent be present with the applicant at the time of application to show parental awareness.

Walk-in Wednesdays

Every Wednesday, starting at 10am, we accept applications on a first-come, first-served basis. We can only accept 20 walk-ins per day. 

  • When you arrive at the library, sign in for a passport time-slot. 
  • Visitors who arrive after the sign-in sheet is full will need to return the next Wednesday, or can make an appointment for Mondays, Tuesdays, or Thursdays.

Schedule an Appointment

Appointments must be made at least 7 days in advance. Walk-in appointments are available on Wednesdays only, from 10am until 3pm. 

Appointments are available at the following times: 

  • Mondays 11-7 (Last appointment at 6:30 p.m.) 
  • Tuesdays from 11-4 (Last appointment at 3:30 p.m.) 
  • Thursdays from 11-4 (Last appointment at 3:30 p.m.)
